fix: exclude unscoped memories from project-filtered recall - #1647
Project-filtered recall could return durable memories with neither explicit project ownership nor a resolvable session. Reject these records when a project filter is present. Preserve explicitly matching memories without sessions, matching legacy session ownership, and unfiltered recall.
Filtered recall now refills full ranked windows instead of losing matches beyond the first window. It uses raw hybrid candidate counts before enrichment drops missing records, and reuses a successful query embedding across windows. The ranked window stops at 1,000 candidates; qualifying records below that budget can leave a partial page.
Filtering and loading run together in bounded parallel batches, starting with the requested page size and capped at ten candidates. Request-local caches prevent repeated session and record loads. A filled page stops later reads. Missing observations do not consume result slots. Directory filtering rejects orphan observations while retaining sessionless durable-memory behavior.
Recovery fixes preserve earlier accepted hybrid results if a later refill fails, keep keyword fallback stable for the request, skip session reads for agent-only filtering, and clear failed embedding promises so later windows can recover vector-only matches. Successful embeddings remain cached.
Validation on ec82f6fce4b20c1e0460263e90700ce313ef3b80:
Current recovery regressions: five failures and 80 passes before the fixes, then all 85 related tests pass with the same test files.
Concurrent hybrid-request changes were independently checked: nine failures against the earlier implementation, then all 80 related cases passed.
npm run build
npm run skills:gen
npm run docs:sync: no generated changes in the follow-up.
npm run skills:check
npm test -- --maxWorkers=4: 4,157 passed, six skipped.
The fresh dependency audit reports nine moderate findings and zero high/critical findings in the unchanged dependency graph. No dependencies changed. Live-server integration and six skipped unit cases were not run. Missing record loads remain cached only within their current request; a later query retries them.
Hosted CI and Release gate require maintainer approval. Vercel requires deployment authorization. The original generated test-count metadata updates are required by AGENTS.md.
Related: #829, a broader project-isolation proposal. Project filtering remains optional.
Summary by CodeRabbit
Bug Fixes
Project-filtered searches exclude unscoped memories when no matching session establishes ownership, as well as memories with unresolved sessions.
Searches can look beyond filtered-out or unavailable candidates to fill the requested result limit. Hybrid searches expand their candidate window as needed, stopping when the limit is met, candidates are exhausted, or the 1,000-candidate ceiling is reached. Results may contain fewer items than requested.
If hybrid ranking fails, searches fall back to keyword results when no hybrid results have been enriched.

Filtered recall now expands candidate windows with bounded reads and reusable embeddings. No actionable merge-blocking regression remains established. The failed-refill test would benefit from asserting that its second ranking call occurs; the unnecessary session-read failure predates this change.

Independent review result: APPROVED, with no substantive findings. A separate read-only Codex reviewer examined this head against base c6c476d. This result is recorded here by the PR author; it is not a GitHub approval submitted by a separate account.
Coverage included all 39 changed files, the project/session ownership flow, recall handlers, authentication contracts, and related tests. The reviewer independently ran the focused regression suite: 24 passed. A byte comparison verified that all 37 generated metadata files contain only test-count updates.
The guard preserves explicit project ownership, matching legacy sessions, unfiltered recall, and filtering before the result limit. The inspected project parameter is an optional caller-selected retrieval filter under the existing service access contract.
The reviewer could not independently execute docs:check because host IPC permissions blocked it and the retry was dismissed by the host. The author-run docs:check passed; those are separate pieces of evidence. The reviewer inspected the full-suite log reporting 4,120 passed and six skipped, without independently repeating that suite. No live memory data or installation details were included in the review or patch.

Reuse the query embedding across expansion windows.
874-875: Minor · Performance & Scalability · Poor tradeoff
Reuse the query embedding across expansion windows.
The expansion loop in registerSearchFunction re-runs the hybrid ranker for each underfilled window. Each run re-embeds the same query, so one filtered recall can make up to four or five embedding calls. The base made one ranker call per request. Sharing the query embedding across windows, or caching it for the request, would keep the extra windows from repeating that work.

Do not end the recall loop on a short hybrid result alone.
The loop stops when the post-enrichment hybrid result is shorter than fetchLimit. Enrichment drops hits that do not resolve, so one dropped hit ends the search before the window grows. Base the exhaustion check on the ranker's candidate count, not the enriched length.

Keyword-fallback candidate loads run in sequential waves of 10.
925-928: Minor · Performance & Scalability · Poor tradeoff
Keyword-fallback candidate loads run in sequential waves of 10.
The hybrid path does not hit this cost. Hybrid results carry their observations, so the loader issues no KV reads for them. The cost applies only when hybrid ranking fails or is unavailable and the search falls back to keyword results. Those results are read from KV in waves of 10 (search.ts:925-951). With filterAgentId, the candidate window can reach 1,000 entries, which means up to 100 waves run in sequence. The base code issued all reads in one Promise.all, so it took about one read latency.
A bounded pool with the same concurrency still takes about candidates ÷ concurrency waves. It removes only the wait for the slowest read in each wave. Shortening the wait requires a higher KV read concurrency limit or fewer candidates loaded per window. In the 100-wave worst case, a read latency above about 150 ms can exceed the 15-second REST proxy deadline (rest-proxy.ts:170) on the standalone MCP recall path.

Reload failed observation loads on later expansion passes.
939-944: Trivial · Functional Correctness
Reload failed observation loads on later expansion passes.
When both kv.get calls reject, the cached promise resolves to null. A later expansion pass that requests the same obsId and sessionId can reuse that null. The candidate then stays dropped for the rest of the request, even if the store recovers. Removing the entry once its load resolves to null lets a later pass reload it. Successful loads remain cached.

Update the observation-cache handling around the `observationCache.set` call so
a load that resolves to `null` removes its entry, allowing later expansion

FIXED in c7db676, retained in the current branch. Both hybrid and keyword test doubles truncate to the requested limit. I verified the current tests exercise window expansion and source exhaustion. The associated inline refill finding is fixed and resolved.
FIXED in 680b719: expansion stops at the documented 1,000-candidate ranked-window ceiling. Regressions assert requested windows of 100, 200, 400, 800 and 1,000 across keyword, hybrid and fallback paths.
FIXED in 5445ccf: an unloadable candidate does not consume a result slot. Loading and filtering continue until the page fills or the bounded source is exhausted. The stale-observation regression passes.
I revalidated both fixes. These review-body comments have no resolvable inline thread; their dispositions are recorded here.
FIXED f8e4651 — Query embeddings are shared through a request-local hybrid context across refill windows. Independent recalls get separate contexts. A regression reproduced two embedding calls before the fix and verifies one call across two windows, plus a new call for a separate request.
FIXED f8e4651 — Exhaustion uses the hybrid ranker's candidate count before enrichment, rather than the shorter list of successfully loaded records. A regression using the real HybridSearch reproduces a missing record hiding a matching memory beyond the first window, then verifies expansion from 100 to 200 retrieves it.
DECLINED the unbounded Promise.all suggestion — It would undo the accepted state-worker overload safeguard in the resolved Devin thread. Candidate filtering and loading remain concurrent within bounded batches of at most ten. A limit-one request starts with one candidate; rejection grows subsequent batches to ten, and a full page stops later work. This avoids needless reads while retaining the documented scan/concurrency bounds. Slow stores can still exceed the proxy deadline in the worst case; unbounded fan-out would trade that for overload rather than guarantee completion.
DECLINED evicting every null cache entry — The cache is local to one recall. Retaining misses prevents repeatedly loading the same stale/unavailable prefix on every expansion. A later recall starts with a fresh cache and retries those records. Recovery during the same request is possible but is not guaranteed; retrying every miss on each expansion weakens the accepted bound on redundant reads. A separate explicit transient-error retry policy would need to distinguish missing records from failures rather than evict both indiscriminately.
The serial-session-read finding is also handled: the concurrent d386c23 commit and its observation regressions are preserved. Filtering and loading now run together, in-flight session reads are shared, and a first qualifying result with limit one reads only its session. Project and directory regressions cover keyword, hybrid and keyword fallback paths.
Validation of integrated head f8e4651: 4,152 tests passed, six skipped; 80 focused isolation/hybrid/layer tests passed. Build, skills generation/check, docs sync/check, evaluation typecheck and git diff --check passed. Each of the three accepted new issues was reproduced by a failing regression before its correction. Audit: nine moderate findings, zero high/critical; dependencies unchanged. Live-server integration and the skipped tests were not run. All four inline threads are resolved. Review-summary findings have no separate resolution control; these replies record their disposition. Hosted checks and deployment still depend on maintainer approval.

Keep graph-matched sessionless memories under cwd filters.

Keep graph-matched sessionless memories under cwd filters.
With a cwd filter, authorizeGraph rejects any record whose session row is missing. Sessionless memories have no session row, so a memory found only through graph retrieval is dropped. The keyword and vector paths keep it. This predates the PR.

Cap refill reads: each window re-reads earlier candidates with up to 1,000 concurrent gets.
881-881: Minor · Performance & Scalability · Poor tradeoff
Cap refill reads: each window re-reads earlier candidates with up to 1,000 concurrent gets.
For a filtered search with default limit, each refill calls hybridRanker again with a larger fetchLimit. enrichResults then re-reads every candidate it returns, with no reuse across windows. The filtered path can issue about 2,400 cumulative observation reads, with a final window fanning out to 1,000 concurrent reads. The base path issued one call of at most about 200 reads for the same limit. Share the enriched observations across refill windows, or enrich in bounded chunks, so each record is read once per request.

Add a unit test for request forwarding through the hybrid ranker.
Add a unit test for request forwarding through the hybrid ranker.
The production ranker in src/index.ts forwards request to hybridSearch.search. No test exercises that forwarding. The assertion at test/recall-layer.test.ts:123 checks a fixture, and the direct HybridSearch test bypasses the adapter. If the argument were dropped, the embedding reuse across refill windows would stop and no test would fail.
Move the adapter into an exported factory so a test can call it without booting the server. The factory can live beside HybridSearch, which src/index.ts already imports.

🟡 Transient failure hides semantic-only memories
When hybridRanker fails on a full, rejected first window, useHybridRanker keeps subsequent windows keyword-only. A recovered ranker never supplies matching vector-only memories, so recall returns no results.
Learn more
A filtered recall expands its candidate window when a full window contains too few accepted results. If the first hybrid call fails, the keyword fallback can return a full window of rejected results, causing another window to be requested. Disabling hybrid ranking for the rest of the request prevents that next window from finding memories available only through vector ranking, even when the ranker has recovered.
Example: A project search has 100 foreign keyword matches and one matching memory found only by its vector. The first hybrid call fails, so the first 100 keyword matches are rejected. The second window runs keyword-only and returns nothing; a successful second hybrid call would return the matching memory.
Recommended fix: Allow a later hybrid attempt after a fallback window underfills, while bounding retry attempts and preserving the existing candidate cap.

Assessed as a follow-up recovery-policy enhancement rather than an isolation regression. If the entire hybrid ranker throws before accepting hits, this request stays in keyword fallback mode. The pre-PR implementation also made one hybrid attempt and then returned keyword results on an exception; it could not return vector-only matches in this example either.
Recoverable embedding-provider failures are handled separately inside HybridSearch. ec82f6f clears a rejected embedding promise so a later hybrid window can retry and recover vector-only matches; that path has a failing-before/passing-after regression.
Retrying the whole ranker after it throws would change the chosen degraded-mode policy. A new recall already starts with hybrid ranking again. I am leaving that stronger within-request retry policy for maintainer decision, not marking this thread fixed.
The reason will be displayed to describe this comment to others. Learn more.
🟡 Later hybrid failure truncates recall results
When hybridRanker fails during a refill after some hits were accepted, recall stops before checking keyword matches. Matching memories in the next keyword window cannot fill the requested page.
Learn more
A filtered recall retries with a larger candidate window when its first window supplies fewer results than requested. If the later hybrid call throws, the immediate break preserves earlier hybrid hits but skips the keyword fallback. Matching keyword records available in the expanded window cannot fill the remaining slots.
Example: A recall requests two memories. Its first hybrid window yields one matching vector-only memory among 99 foreign memories. The expanded hybrid call fails, while the keyword index has another matching memory at position 101. Recall returns one memory instead of two.
Recommended fix: Retain accepted hybrid hits on a later failure, then search the expanded keyword window for additional eligible records without replacing or duplicating those hits.
Was this helpful? React with 👍 or 👎 to provide feedback.
The reason will be displayed to describe this comment to others. Learn more.
Assessed as a follow-up recovery-policy enhancement. The current policy preserves the already accepted hybrid hits and returns a partial page when a later hybrid refill throws. That restores the pre-PR result for this example and fixes the original regression where fallback erased valid earlier hits.
Appending keyword hits would introduce a mixed-mode merge, deduplication and ordering policy. The existing response does not promise a full page during a ranking dependency failure. The conservative partial-result policy is explicit in the PR description and covered by the hybrid-only-hit regression.
I am leaving the proposed stronger fallback completion policy for maintainer decision, not marking this thread fixed. The 1,000-candidate ceiling, project/session filters and request-local caches remain unchanged.
Please review current head ec82f6f. It fixes the three recovery findings, with five failing cases before correction and all 85 related tests passing afterward. Full suite: 4,157 passed, six skipped.

Skip the session read when the memory project already matches.
With projectFilter set, src/functions/search.ts:918 still awaits loadSession for every hit. A memory whose explicit project equals the filter does not need its session to pass the filter. The test preserves accepted hybrid hits when a later refill fails increments calls in the ranker but never reads it. The current assertions also pass when no second ranker call occurs. In that case, the first window already returns mem_vector_only, and keyword search is not reached. The test does not confirm that the refill runs before it checks preservation.
