fix(claude): close the uncoded overflow gaps left after #8128 - #8129
#8128 added isContextOverflowError. With it, an uncoded context-window refusal answers 400 prompt is too long on the translated stream, the non-stream error branch and the failed-JSON branch. I rebased this PR onto dev after #8128 and dropped my overlapping version, so it builds on isContextOverflowError unchanged. Testing dev after #8128 found four shapes that still reach Claude Code as a retry or a 502. This PR fixes them:
A 5xx overflow is sent three times. Before the Claude path classifies an error, the shared transient retry (fetchWithTransientRetry in src/lib/upstream-retry.ts) resent a 500 Your input exceeds the context window twice. For a session past an 872K window, that is about 2.6M input tokens for a refusal that cannot succeed. The retry now returns a 5xx at once when its body is a definite overflow verdict (isDefiniteContextOverflow).
The check reads a clone of the body, capped at 16 KB and 250 ms, so the caller still receives the body untouched. A slow or endless body, or an event stream, gets the normal retry.
A 5xx throttle worded only as Too many tokens, please wait is still retried.
An overflow coded upstream_server_error returns 502. The shared classifier gives that code to every 5xx, so it says nothing about the cause. It now counts as a generic failure code alongside server_error.
The direct Messages encoder checks only the code.src/protocols/encoders/messages.ts already used the 400 status from anthropicFailedStatus, but it attached context_length_exceeded only to coded errors. It now uses the same isContextOverflowError verdict as the translated stream, so an uncoded overflow gets the code that Claude Code compacts on.
A plain-text body isn't classified. In the non-stream branch, a non-JSON 5xx overflow such as text/plainYour input exceeds the context window… went to the JSON-parse catch and became a retryable 529. It now answers 400 with the upstream text as its message. 401, 402, 403 and 429 keep their status (402 is a billing refusal, per CodeRabbit review).
Review findings addressed
Codex P1 (owner docs): the three owner documents above now describe the retry stop, the added generic code, the plain-text verdict and the encoder's shared helper.
CodeRabbit (HTTP 402): a plain-text 402 keeps its billing status.
Codex P2 (validation metadata) and CodeRabbit (outer HTTP status before body status): both were raised on the pre-rebase head's helper, which this PR no longer has. On this head, the classifier is fix(claude): answer uncoded context-window refusals as prompt too long #8128's isContextOverflowError, which returns false for {code:"invalid_value",param:"tools[0].function.name",message:"…maximum context length"}. The non-stream branch excludes 401/402/403/429 by the real HTTP status before the verdict. The retry stop uses isDefiniteContextOverflow, which rejects parameter-validation records.

Full-suite exception: I didn't rerun the full bun run test on this head. On the previous head, the full suite on this machine had 234 failures in 35 files outside the changed areas: local port conflicts with a running opencodex instance, and the lab, management and bearer-admission suites. Running those same 35 files on unmodified dev gave the identical 234 failures, so they are host baseline failures, not regressions. PR CI and maintainer review remain required.
Tests added
tests/lib/upstream-retry.test.ts:
a JSON 5xx overflow is sent once and returned untouched
a plain-text 5xx overflow is sent once and returned untouched
a 5xx body that never ends does not stall the check and is retried
a 5xx Too many tokens, please wait throttle is still retried
an upstream_server_error overflow in the translated stream ends in a prompt-too-long 400 frame
a plain-text 500 overflow answers 400 with the upstream text
a plain-text 500 that is not an overflow stays 529
a plain-text 402 or 429 that mentions the context window keeps its status
tests/responses/protocol-direct-encoders-messages.test.ts: on the direct encoder, an uncoded overflow and an upstream_server_error overflow each become a prompt-too-long 400, with stream parity against the translated path.

Walkthrough
The changes classify additional upstream context-overflow errors in Messages paths and check eligible transient responses for definite overflow before retrying. Tests cover error mapping, excluded statuses, bounded body inspection, and retry behavior.
upstream_server_error can proceed to message-based overflow classification. Messages encoders classify qualifying errors as context_length_exceeded. The Claude Messages server checks eligible plain-text error bodies and maps qualifying overflow text to a 400 response. Tests cover these mappings and excluded statuses. The runtime and protocol-path documentation reflects the related classification and helper changes.
Check transient responses before retry src/lib/upstream-retry.ts, tests/lib/upstream-retry.test.ts, structure/data-planes/error-detail.md
Before retrying a transient response, the retry path inspects a cloned body for up to 16 KiB or 250 ms, except for event-stream responses. It returns the original response when the check identifies definite context overflow. Tests cover overflow, slow or incomplete inspection, and “too many tokens” text that remains retryable. The documentation describes these conditions.

Merge Risk: Low · up to 9424d
The overflow fixes have bounded remaining issues: oversized chunks incur unnecessary decoding, and architecture documentation overstates inspection guarantees. These support merging with owner awareness and localized follow-up; the JSON billing-status inconsistency is pre-existing.

Classify a status-429 adapter error as a rate limit, not a context overflow.
On the direct Messages route, classifyError checks its context-window text rule before the status-429 branch. An adapter error with status: 429 whose message contains context window or too many tokens is classified as context_length_exceeded. The status reaches classifyError through adapterFailureFromEvent, but the error case in src/protocols/encoders/adapter-events.ts:785-789 copies only failure.error into the terminal event. The encoder then returns a 400 prompt-too-long error. Claude Code compacts the session instead of backing off, which conflicts with the stated 429 behavior at src/claude/outbound.ts:141-142.
Gate the text rule on the status that classifyError already receives. Coded overflow still takes precedence.

expectContext accepts either an SSE error frame or a JSON 400 for every stream=true row. For an upstream response.failed sent as SSE, the route returns status 200 with text/event-stream. A regression that returns a JSON 400 for these two rows would still pass. Add an explicit status and Content-Type check for them. The JSON branch stays in place for the non-2xx and failed-JSON rows.

fix(claude): classify uncoded context overflow as prompt too long (#8127)
fix(claude): close the uncoded overflow gaps left after #8128
Nov 3, 2026
lidge-jun#8128 answers an uncoded context-window refusal as prompt too long on the
translated stream, the non-stream error branch and the failed-JSON
branch. Four shapes still reached Claude Code as a retry or a 502:
- The shared transient retry resent a 5xx overflow twice before the
Claude path classified it. It now returns a 5xx whose body is a
definite overflow verdict without resending. The body is peeked from a
clone within a byte and time bound, so the caller keeps the untouched
body and a slow or endless body still gets the normal retry; a 5xx
throttle worded only as "too many tokens" is still retried.
- upstream_server_error, the code the shared classifier gives every 5xx,
now counts as a generic failure code like server_error.
- The direct Messages encoder attaches context_length_exceeded with the
same isContextOverflowError verdict as the translated stream.
- A plain-text (non-JSON) overflow body now answers 400 with the upstream
text as its message.

…ment the gaps
- A plain-text 402 that mentions the context window keeps its billing
status instead of becoming a prompt-too-long 400 (CodeRabbit review).
- Update the owner docs for the changed areas (Codex review):
structure/runtime.md for the translated-lane verdict additions,
structure/data-planes/error-detail.md for the retry stop in
src/lib/upstream-retry.ts, and structure/data-planes/protocol-paths.md
for the shared helper the direct Messages encoder now uses.

Exclude HTTP 402 from the uncoded JSON overflow rewrite.
An uncoded JSON HTTP 402 whose message asserts a context overflow is rewritten to 400 context_length_exceeded. The plain-text branch below this guard already keeps 402. Adding 402 to the JSON status list makes both branches agree. A coded context_length_exceeded still counts, as documented.
Add a JSON case to tests/claude-integration/claude-uncoded-context-overflow.test.ts that asserts HTTP 402 keeps its status. The existing 402 test covers only the plain-text body.

Cap the overflow peek by bytes before decoding each chunk.
peekBodyText decodes each full chunk from reader.read() and truncates the text only after the loop. A single large chunk is therefore fully decoded even though the function documents a byte bound of maxBytes. Slice each chunk to the remaining byte budget before decoding it.

Codex control-plane ids are defined in the dependency-free `src/codex/control-plane-models.ts`; `src/router.ts` routes these exact ids unchanged to canonical OpenAI, retaining the existing error when no OpenAI provider is enabled. Async runtime persistence awaits the fresh [selection admission](codex-cli-update.md#worker-realm-and-the-selection-commit-window), rechecks observed selection/epoch and caller-currentness changes after that await, and returns detached frozen results; signal cancellation makes the persistence wrapper return null without writing, while raw async resolver rejections and exceptions without signal cancellation retain their rejection behavior; bounded exec rejects promptly and releases output readers on deadline or abort, independently awaits actual child exit, and escalates SIGTERM to SIGKILL after the shared 2-second grace (Windows termination is forceful). Known Codex.app and ChatGPT.app commands at any location or alias follow the [execution trust contract](clients/chatgpt-desktop.md#codex-app-execution-trust); automatic discovery also requires matching bundle identity; the existing ChatGPT bundle trust policy (safe ownership, permissions, non-symlink paths, and OpenAI signatures for both the bundle and the exact executable) is enforced at every execution boundary in src/codex/exec-boundary.ts before the resolved canonical target is spawned, not once at selection. Version probes retain that execution's bundle classification for selection, so a bare PATH alias of an app cannot supersede a discovered persisted runtime; selection does not resolve PATH again after the probe. Stored and displayed command spelling remains unchanged, and this observation grants no trust to a later execution. Claude overflow normalization examines at most 64 Ki characters, extracts stated context/request counts in separate forward scans, retains the bounded rewritten prefix, and leaves other error classifications intact; the managed native Messages SSE route follows the same contract (`tests/claude-integration/claude-prompt-too-long.test.ts`). On the translated Messages-to-Responses lane, `isContextOverflowError` in `src/claude/outbound.ts` decides overflow for the streamed `response.failed` handler, the non-stream error branch and the failed-JSON branch: a coded `context_length_exceeded` always counts; an uncoded or generic-coded (`server_error`, `upstream_error`, `api_error`, `internal_error`) refusal counts only on wording that asserts the request exceeds a size limit, never with a 401/403/429 status, a rate-limit or credential type, throughput wording or another specific code, and then answers 400 `prompt is too long` so Claude Code compacts (#8127, `tests/claude-integration/claude-uncoded-context-overflow.test.ts`). Optional native desktop observation follows the [Codex Desktop startup gate and awaited teardown](clients/codex-desktop.md#proxy-startup-preference). Claude Code tier defaults are projected by `src/claude/model-default-settings.ts` into user settings only after explicit preview/apply, independently of launcher environment injection; see the [management contract](gui-and-management-api.md). The explicit projection pairs selected aliases with readable labels from `src/claude/context-windows.ts`, preserving custom labels and clearing only a label matching its current alias when an explicitly selected tier becomes a non-alias; unset tiers retain both settings model IDs and labels. No startup or periodic sync writes these defaults.

On the translated Messages-to-Responses lane, `isContextOverflowError` in `src/claude/outbound.ts` decides overflow for the streamed `response.failed` handler, the non-stream error branch and the failed-JSON branch: a coded `context_length_exceeded` always counts; an uncoded or generic-coded (`server_error`, `upstream_error`, `api_error`, `internal_error`) refusal counts only on wording that asserts the request exceeds a size limit, never with a 401/403/429 status, a rate-limit or credential type, throughput wording or another specific code, and then answers 400 `prompt is too long` so Claude Code compacts (#8127, `tests/claude-integration/claude-uncoded-context-overflow.test.ts`) `upstream_server_error`, the code the shared classifier gives every 5xx, is generic there too. The non-stream branch applies the same verdict to a plain-text (non-JSON) body, excluding 401/402/403/429, and answers with the upstream text as the message; the direct Messages encoder attaches `context_length_exceeded` with the same verdict.

Correct the 5xx classifier sentence in structure/runtime.md.
The sentence says upstream_server_error is the code the shared classifier gives every 5xx. A 503 does not get that code. The classifier returns server_is_overloaded for a 503, so the sentence should name that exception. The same inaccurate claim appears in the comment at src/claude/outbound.ts:136.
Suggested wording for the doc: "upstream_server_error is the code the shared classifier gives generic 5xx responses; a 503 gets server_is_overloaded." Apply the same change to the comment at src/claude/outbound.ts:136.

Review comment at @structure/runtime.md at line 3:
Update the 5xx classifier sentence in the runtime documentation and the
corresponding comment near isContextOverflowError to state that
upstream_server_error applies to generic 5xx responses, while 503 uses
server_is_overloaded.

Thanks — the rebase onto #8128 is clean, and the regression tests do fail on dev and pass with the fix. I have approved the fork CI runs (Cross-platform CI, Catalog async contracts) on 9424d4bd6; merge needs them green on this head. The remaining notes are minor and don't block:
src/lib/upstream-retry.ts:739-743 — peekBodyText decodes each full chunk and truncates only after the loop. A 5xx body delivered as one large chunk (for example a buffered multi-MB HTML error page) is decoded in full despite the 16 KiB bound. Please slice each chunk to the remaining byte budget before decoder.decode (as CodeRabbit suggested).
src/lib/upstream-retry.ts:830 — the overflow peek runs before the opts.abortSignal?.aborted check, so an already-aborted caller can wait up to 250 ms, and that wait also counts toward slowAttemptMs. Moving the abort check above the peek keeps the same contract, because that branch returns res without cancelling its body.
structure/data-planes/error-detail.md:211-216 — the text says "at most 16 KiB" and says a slow or endless body keeps the normal retry. The code keeps up to 16,384 UTF-16 code units, and a body that sends overflow wording within 250 ms and then stalls is not retried. Please reword it to match the code.
src/claude/outbound.ts:136 and structure/runtime.md:3 — "upstream_server_error is what the shared classifier gives every 5xx" is inaccurate: classifyError gives server_is_overloaded to a 503 or to "overloaded" wording (src/lib/errors.ts:571-579). Please say "generic 5xx". The runtime.md edit also dropped the period after "…claude-uncoded-context-overflow.test.ts)", which joins two sentences.
Optional, for consistency: src/server/claude-messages.ts:1301 (JSON branch) still excludes only 401/403/429. A JSON 402 with an uncoded overflow message becomes a 400 context_length_exceeded, while the plain-text branch at :1306 keeps 402. Using the same 401/402/403/429 exclusion in both branches would match the CodeRabbit 402 fix.
The branch is also 14 commits behind dev. The merge is clean, but please rebase onto the current tip when you push the fixes above.

Follow-up to #8128 for #8127.
#8128 added
isContextOverflowError. With it, an uncoded context-window refusal answers 400prompt is too longon the translated stream, the non-stream error branch and the failed-JSON branch. I rebased this PR ontodevafter #8128 and dropped my overlapping version, so it builds onisContextOverflowErrorunchanged. Testingdevafter #8128 found four shapes that still reach Claude Code as a retry or a 502. This PR fixes them:fetchWithTransientRetryinsrc/lib/upstream-retry.ts) resent a 500Your input exceeds the context windowtwice. For a session past an 872K window, that is about 2.6M input tokens for a refusal that cannot succeed. The retry now returns a 5xx at once when its body is a definite overflow verdict (isDefiniteContextOverflow).Too many tokens, please waitis still retried.upstream_server_errorreturns 502. The shared classifier gives that code to every 5xx, so it says nothing about the cause. It now counts as a generic failure code alongsideserver_error.src/protocols/encoders/messages.tsalready used the 400 status fromanthropicFailedStatus, but it attachedcontext_length_exceededonly to coded errors. It now uses the sameisContextOverflowErrorverdict as the translated stream, so an uncoded overflow gets the code that Claude Code compacts on.text/plainYour input exceeds the context window…went to the JSON-parse catch and became a retryable 529. It now answers 400 with the upstream text as its message. 401, 402, 403 and 429 keep their status (402 is a billing refusal, per CodeRabbit review).Review findings addressed
isContextOverflowError, which returns false for{code:"invalid_value",param:"tools[0].function.name",message:"…maximum context length"}. The non-stream branch excludes 401/402/403/429 by the real HTTP status before the verdict. The retry stop usesisDefiniteContextOverflow, which rejects parameter-validation records.Verification
